MCP 2026-07-28 support in 14hrs while retaining legacy compatability

AsDecided now supports MCP 2026-07-28, delivering a new stateless lifecycle for current hosts while retaining compatibility with initialize-based clients through MCP 2025-11-25. The update includes server/discover, per-request protocol metadata, standard MCP HTTP routing headers, explicit result types, cache hints, and JSON Schema 2020-12-compatible tool schemas, governed internally via ADR-121.
