COVID-19, initially focused on prescriptions and helping patients stay on top of their medications and appointments.\n\nOver time, we realised that the bigger problem was not simply access to prescriptions or consultations.\n\nIt was continuity of care.\n\nHow do we know what is happening to a patient's health between two hospital visits?\n\nThat question led us toward building a technology infrastructure that connects the patient, medical devices, AI and healthcare professionals into one continuous loop.\n\nWe call this the Presibo Care Loop.\n\nThe Care Loop\n\nAt a high level, the architecture looks like this:\n\nPatient → Device → Data → AI → Risk Detection → Triage → Healthcare Professional → Intervention → Patient\n\nThe loop then continues.\n\nInstead of waiting for a patient to arrive at a hospital with a problem, the system is designed to continuously collect health signals and surface situations that may require attention.\n\nConnected Medical Devices\n\nOne of the biggest steps in our evolution has been moving beyond software.\n\nWe are developing connected blood pressure monitors and glucometers that can communicate with the Presibo platform.\n\nA reading taken from a device can be transmitted into the patient's digital health record, allowing the platform to build a longitudinal picture of the patient's health rather than relying on isolated measurements.\n\nThis changes the question from:\n\n«\"What is the patient's blood pressure today?\"»\n\nto:\n\n«\"What has been happening to this patient's blood pressure over time?\"»\n\nThat distinction is extremely important in chronic disease management.\n\nThe AI Layer\n\nRaw health data by itself is not particularly useful.\n\nThe challenge is turning thousands of individual readings into meaningful information.\n\nOur AI layer is designed to analyse patient data, identify patterns and flag readings or trends that may require attention.\n\nFor example, instead of simply storing:\n\nBP: 168/104\n\nthe system can look at the patient's history, previous readings and other available signals to determine whether the reading represents an unusual change that should be escalated.\n\nThe goal is not to replace doctors.\n\nThe goal is to help healthcare professionals see the right information at the right time.\n\nIntelligent Triage\n\nOnce a concerning signal is identified, it can enter our triage and escalation workflow.\n\nThe severity of an event determines the appropriate response.\n\nA relatively low-risk situation may result in a notification or recommendation.\n\nA more concerning situation may be routed to a nurse or health attendant.\n\nHigher-risk situations can be escalated to doctors or specialists.\n\nPotential emergencies require an entirely different response pathway.\n\nThis creates a technology-driven hierarchy of care rather than sending every patient through the same workflow.\n\nThe Human Layer\n\nOne principle is extremely important to us:\n\nAI should assist healthcare professionals, not remove them from the loop.\n\nA doctor should be able to see the patient's relevant information, understand why an alert was generated and take the appropriate clinical action.\n\nPresibo therefore connects technology to people.\n\nThe system is not simply:\n\nDevice → AI\n\nIt is:\n\nDevice → AI → Triage → Human
